Transition Assessment
Transition Planning should be considered at age 14, or beginning in 8th grade in Livingston County, and must be completed for a student's IEP within the year they turn 16. SPP13 is a compliance factor that requires specific areas of transition to be reflected within the IEP process.

An Age Appropriate Transition Assessment must be completed and should encompass the student's interests, abilities, preferences and strengths, utilizing multiple data sources.
